Why Facilities Need OBS
We Are Your Facility’s Behavioral-Health Infrastructure
OBS installs, manages, and operates the complete behavioral-health system inside your community — so your staff never has to. We screen every resident, coordinate all provider visits (both in-person and telehealth), centralize scheduling, and automate Title 22 compliance through one unified workflow.
Every resident receives a dedicated in-room telehealth device, full behavioral-health screening, ongoing support, and easy access to therapy without travel or staff involvement. All services are billed through Medicare, Medicaid, and most insurance plans, with no cost to the facility.
Our clinicians provide care.
OBS handles everything else.
We replace fragmented behavioral-health processes with one integrated, compliant, fully managed infrastructure — built around your residents and your regulatory requirements.
Title 22 Requirements (What You Must Manage)
Facilities must maintain:
Documented mental-health status and care notes (both at intake and throughout stay)
Ongoing psychosocial and mood monitoring
Behavioral and cognitive observations
Dementia-related behavior tracking
Regular mental-status checks
Updated records for each resident receiving behavioral support
Penalties for Not Meeting Standards
Non-compliance is treated as a serious violation:
CA facilities have received six-figure fines on Title 22 violations
CDSS is increasing enforcement and fining or closing facilities lacking behavioral-health tracking
Missing or incomplete notes count as a violation — even if care was provided
OBS prevents these issues before they occur, protecting your license and keeping you inspection-ready.

What We Do
OBS provides the infrastructure and system management your facility needs to meet Title 22 behavioral-health requirements.
-
Quick behavioral-health screenings for new and existing residents.
-
Licensed clinicians perform full assessments for residents who screen positive.
-
We confirm coverage and connect residents to enrollment support when needed.
-
We provide the devices, set them up and manage them. Residents only tap once to join sessions.
-
OBS provides your facility with a daily list of all scheduled sessions — telehealth or onsite — including times, providers, and resident status.
This eliminates undocumented visits and strengthens Title 22 compliance.
-
Telehealth sessions for residents who need continued care.
-
Yearly behavioral-health updates required under Title 22.
-
We coordinate everything with licensed clinicians so your staff doesn’t have to.
How OBS Works
OBS is your behavioral-health infrastructure — screening every resident, centralizing all provider visits, and automating Title 22 compliance.
-
We install the full system: in-room devices, kiosk-mode setup, telehealth access, scheduling platform, and Title 22–compliant tracking.
Nothing for staff to manage.
-
You choose the screening day(s).
OBS conducts a quick, universal mental-health screening so every resident gets documented — even those already seeing outside therapists or with dementia (required for Title 22 compliance). -
We reach out to each resident’s existing therapist, psychologist, or wellness provider to:
Move all future in-person visits into the OBS scheduling system
Ensure every visit is tracked and timestamped
Ensure the facility becomes fully Title 22 compliant
Your staff no longer has to coordinate anything.
-
We verify insurance, coordinate benefits specialists for uncovered residents, and set up all telehealth access for each resident.
We leave properly configured devices onsite for therapy connections.
-
We schedule and manage all therapy sessions (onsite + telehealth).
All sessions are documented by the clinicians and routed through OBS for Title 22 tracking.
